Abstract
Background: We present the evolving analysis of fibrinogen (Fbg) after a standard laparoscopic biliopancreatic diversion (BPD). Laparoscopic biliopancreatic diversion (BPD) is a safe and effective treatment for refractory morbid obesity. Given the pro-inflammatory effect in both adipose tissue and surgical aggression, the purpose of this study was to assess the response of Fbg, an acute-phase protein, to BPD. Methods: Scopinaro's laparoscopic BPD was performed in 98 patients, with gastric preservation in those older than 35 years or in those with eradication of Helicobacter pylori. A clinical and analytical follow-up (including plasma Fbg concentration) was performed at 1, 3 an 6 months, and a t 1, 2 and 5 years postoperatively. Fbg determination was measured by a functional method (Von Clauss). Results: Fbg showed a statistically significant decrease from the first year after laparoscopic BPD, and maintained a statistically significant correlation with coagulation parameters. Conclusions: We were not able to demonstrate an acute-phase response characteristic of a surgical aggression (laparoscopic BPD) through plasma Fbg concentration measurement.
