Comparison of visual and quality of life outcomes after implantation of a trifocal diff ractive lens or a bifocal hybrid lens

Resumo

PURPOSE: To compare the visual and quality of life outcomes within the fi rst 3 months after bilateral implantation of either a Versario diff ractive bifocal or FineVision trifocal diff ractive intraocular lens (IOL). SETTING: Hospital Oftalmológico Quirón A Coruña (private clinical practice). METHODS: Th is prospective, randomized, examiner-masked study included 120 eyes of 60 patients (median 60.13 ± 7.08 years of age). All patients underwent bilateral cataract surgery with bilateral implantation of either a Versario IOL or FineVision IOL between January 2012 and July 2014. Monocular distance, intermediate and near (both uncorrected and distance-corrected) visual acuity (VA) outcomes, contrast sensitivity (CS), subjective visual symptoms, and quality of life (QoL) were evaluated 3 months after surgery. RESULTS: FineVision IOL achieved better postoperative VAs (uncorrected distance: p < 0.001, corrected distance: p < 0.013, uncorrected intermediate: p < 0.001, uncorrected near: p = 0.016, and distance-corrected near: p = 0.004), except for distance-corrected intermediate VA, where both IOLs showed excellent outcomes. CS results remained within the normal range. With respect to postoperative QoL, the FineVision group reported better eyesight, while the Versario patients described less discomfort and were more satisfi ed with what they could accomplish with their eyesight. However, 20% of the latter reported extreme diffi culty driving at night. CONCLUSION: Th e FineVision trifocal IOL provides better distance, intermediate and near VA than the Versario IOL, which provides better comfort and patient satisfaction. Th e excellent intermediate VA suggests the ability of patients to develop a third intermediate focal point after implantation of both IOLs.
