Resumo

Objective: to describe the patients attended by the PAC (continued care point) of Vigo in their home, functional situation, baseline clinical situation and use of health resources. Reason for urgent home care, new diagnoses and treatment. DESIGN: Descriptive, retrospective study. The data was collected from the computerized medical record. Patients attended at home during the year 2015 were included. Results: There were 3,726 urgent homes. Average age of 77.7 years. 58.4% were 80 or more years old. The majority, women. 43.6% immobilized or dependent. 46% did not include the functional situation. Two thirds had between 1 and 4 chronic health problems, the most frequent being hypertension. 55.9%, polymedicated. The most frequent reason for attention was agitation / aggression, followed by altered level of consciousness. The most frequent new diagnosis was respiratory infection. A third of the patients were referred to the hospital, twice as many in emergencies. Conclusions: The domiciles served by the PAC of Vigo corresponded mainly to elderly people and women. The functional situation of the patients was poorly reflected in the medical records. The greater the age, the greater number of problems and prescribed drugs. The most frequent chronic diseases were hypertension and dyslipidemia. The most frequent reason for attention in young people was aggressiveness/agitation and the alteration of the level of consciousness the first in older.
