Conocimientos y cuidados de la urticaria facticia o dermatográfica. Revisión clínica

Resumen

La dermatografía sintomática (urticaria facticia) es un tipo de urticaria crónica inducible que afecta hasta a un 5% de la población. Se caracteriza por la aparición de erupciones y ronchas en la piel, acompañadas o no de prurito, como resultado de la aplicación de un roce, rascamiento, frotamiento o fricción sobre ella. Los factores causales pueden ser múltiples, pero se desconoce la causa del origen. Aunque la enfermedad fue identificada hace casi 100 años, los estudios actuales ponen de relieve que es una patología todavía desconocida, en comparación con otras urticarias. El objetivo del estudio fue hacer una actualización clínica para conocer qué terapias y cuidados se están aplicando, y cuál es la efectividad que presentan. A través de una revisión bibliográfica en PubMed, ClinicalKey y UpToDate, se realizó una síntesis de los hallazgos, así como una exposición narrativa de la evidencia disponible. Como conclusión, no hay una pauta de tratamiento estándar para el dermatografismo. En casos leves se suele recomendar el uso de protectores emolientes, como la vaselina. En los demás casos se suele optar por la administración de antihistamínicos H1 de 2ª generación y/o H2 por vía sistémica, en combinación con anticuerpos monoclonales, como el omalizumab. Se precisan más estudios que aporten una mayor evidencia científica, tanto en el campo de la prevención como en el avance de terapias más eficientes. Los cuidados de enfermería están orientados al autocuidado, con el propósito de mejorar la calidad de vida del/la paciente, y dar apoyo biopsicosocial.
